Shared Living Services
We provide Shared Living Services for individuals who live in the central and southern regions of New Hampshire. We offer combined services, both Residential Habilitation and Community Participation.
This program is based on a “Shared Living” approach to provide quality care and support for adults with intellectual and developmental disabilities receiving services through the Bureau of Developmental Disabilities (BDS) and local Area Agencies. Shared Living programs are a housing option where people in the community open their doors to an individual with developmental disabilities and offer them a safe, loving place to call home. It is a very effective support system that gives individuals the opportunity to live a more interdependent life with a sense of belonging to their community.
Nursing Services
We offer specialized Developmental Disabilities and Intellectual Disabilities Nursing services that adhere to the He-M 1201 State Regulations, ensuring the highest quality of care and safety. Our Nurse Trainers provide consultation, education and training for home care providers and teams in medical conditions, treatments, medication administration and healthcare coordination. We ensure that all home care providers are thoroughly oriented to the health and safety needs of the individuals and are continuously updated on changes to their health conditions.
We perform quarterly quality assurance checks to ensure medication is being administered in accordance with established physician’s orders, regulations and protocols. Our commitment to excellence includes 24/7 emergency support and triage to handle any urgent medical concerns, providing peace of mind to home care providers, individuals and their families.
